The THRIVE Tour is Elev8 Health's dynamic, community-centered initiative dedicated to improving maternal health outcomes, particularly among African American and American Indian/Alaska Native women—communities disproportionately affected by high maternal mortality and morbidity rates. Recognizing that the U.S. has the highest maternal mortality rate among developed nations, with over 80% of these deaths being preventable, the THRIVE Tour brings together mental health professionals, birth workers, medical providers, families, and community members to serve as the "village" of support. This collaborative care model is established in each city through engaging events, expert-led panels, and powerful storytelling. We tackle critical issues such as self-advocacy, postpartum depression, and the importance of essential support systems such as doula and midwifery services.
On the THRIVE Tour, dads and dads-to-be receive support through meaningful, private discussions about their needs and challenges. They are also provided with tools to enhance their mental health and maximize their potential as fathers. Our goal is to empower communities with practical tools, resources, and knowledge, nurturing a supportive village that dismantles systemic barriers and ensures healthier, thriving futures for mothers, fathers, children, and entire families.

Mom's
Lounge
Moms will engage in small group discussions on maternal health, lactation, and breastfeeding.
Dad's Den
Dads will attend small group discussions on paternal health and mental health.
Kid's Corner
Toddlers will be able to play and engage in fun educational activities.
Panel
A panel of maternal health experts that educates pregnant and postpartum women on how to advocate for themselves in health care settings, understand support systems that are available through doula and midwifery services, and how to recognize the signs of postpartum depression.

Discerning the Times, Transforming the Future
The Faith and Philanthropy Collective (formerly known as the 100 Million Reach Collective) is a bold and unprecedented initiative uniting influential faith leaders across the nation, whose collective voices has the potential to reach and inspire approximately 100 million individuals. Driven by a shared passion for improving public health outcomes, the Collective addresses the social determinants of health—housing, education, economic stability, healthcare access, and more—that profoundly shape life in underserved communities. It seeks to reimagine how faith-based institutions collaborate strategically with philanthropic organizations, public-private partnerships, and foundations, unlocking innovative and transformative solutions. By leveraging the deep trust, local knowledge, and extensive networks of faith communities, along with these powerful partnerships, the Collective aims to create sustainable, systemic change, empowering healthier, thriving communities nationwide.

Elev8 Health's "Mental Health is Health" initiative addresses the critical intersection of social determinants and mental health outcomes in underserved communities. Recognizing that nearly 1 in 5 U.S. adults experiences mental illness each year, yet fewer than half receive adequate support, our initiative creates spaces for authentic dialogue, digital storytelling, and impactful advocacy. Mental health isn't only individual—it's about the quality of our communities. By capturing real experiences, highlighting systemic barriers, and engaging community and faith leaders, we aim to dismantle stigma, drive policy changes, and foster accessible, culturally competent mental health care for all. The Health & Healing Collaborative
Reaching & Treating New Yorkers with Serious Mental Illness (SMI)
Faith-Anchored | Clinically Integrated | Capacity-Building
The Health & Healing Collaborative is a faith- and community-anchored mental health initiative designed to expand access to care for individuals living with Serious Mental Illness (SMI). In partnership with the American Psychiatric Association Foundation, the program strengthens clinical capacity while embedding trusted peer navigation within houses of worship.
By positioning faith institutions as access points—not treatment providers—the Collaborative bridges gaps between clinical systems and communities too often left behind.
Program Model
The Collaborative combines three core strategies:
Clinical training and coordination to strengthen care for high-need populations
Mental Health Peer Navigators supported by Elev8 Health’s Care Navigation team
Faith-based outreach and Grace in Action community events to reduce stigma and connect individuals to care
Together, this model expands access, builds workforce and capacity-building pathways, and creates sustainable connections between care systems and community trust.

Social Determinants of Health

Social determinants of health (SDOH) are the underlying conditions in which people are born, live, work, and age, influencing every aspect of health and well-being. Research shows that up to 80% of health outcomes are determined by these social, economic, and environmental factors. Public health crises such as maternal mortality, mental health disparities, and chronic diseases cannot be effectively addressed without confronting the inequities embedded in housing, education, income, healthcare access, and community support systems. At Elev8 Health, we educate communities and stakeholders on the critical intersection of SDOH and public health, advocating for systemic changes to create lasting, meaningful improvements in health outcomes.
At Elev8 Health, we take a multi-faceted approach to addressing social determinants of health (SDOH) by actively bridging gaps between communities, healthcare providers, and policymakers. We collaborate with organizations to develop innovative funding models that support maternal health, mental wellness, and chronic disease prevention.
